In consideration of La Liga allowing minor participantes to participate in activities, events, retreats and trips, I, the undersigned, hereby release, forever release and agree to hold harmless La Liga / Nueva Vida, its pastors, administrators, directors, agents, officers, members, volunteers, employees, other participants, sponsors, advertisers (collectively herein the "CHURCH"), and if applicable, owner and lessor of premises on which the activity is conducted (each one considered one of the "RELEASES" herein) from any liability, claim and demand for accidental personal injury, illness or death, as well as property damage and expenses, of any nature, which may be incurred by the undersigned and the participant while participating in the activities. I, the parent or legal guardian of this minor participant, hereby grant permission for the minor participant to fully participate in activities that occur both on and off church premises. In addition, I, on behalf of my minor participant, assume all risks of accidental personal injury, illness, death, damages, and expenses as a result of participation in recreational and employment activities involved therein. The undersigned also agrees to hold harmless and indemnify said CHURCH for any liability sustained by said Church as a result of negligence, willful or willful acts of such minor participant, including expenses incurred in connection therewith.

NOTE: LA LIGA WILL FOLLOW LIGHTING PROTOCOLS IF WEATHER BECOMES A FACTOR DURING GAME TIMES.

In the event of an emergency, an effort will be made to contact the parent / guardian and / or emergency contact with the information provided on this form. In the event that contact cannot be made, La Liga / Nueva Vida staff and / or adult volunteers in whose care your child has been entrusted will assume responsibility for transporting your child to a medical facility if deemed necessary. In this same circumstance(s) mentioned above, an adult, in whose care your child has been entrusted, may choose to obtain appropriate treatment, hospitalize, and order that your child be provided with any emergency X-rays, diagnoses, or anesthetic, medical, surgical or dental treatment and hospital care, under the general or special supervision and advice of any licensed physician or dentist. The parent / guardian shall be responsible for paying all costs and expenses incurred in connection with such medical and dental services rendered to the aforementioned child.

I, on behalf of my minor participant, understand that there may be photographs, video footage, and other recordings taken of the minor participant and all other attendee events during church-related activities, and that this photograph, video, and other recordings may be used for promotional purposes, including publication on the Internet. I grant La Liga / Nueva Vida permission to use and reuse, publish and repost, and to modify and alter any photographs, videos and other recordings of related events. I also waive my right to inspect or approve any editorial text or copy used in connection with the photographs, video footage and other recordings and hereby release and release La Liga / Nueva Vida from any and all claims arising out of the use of photographs, video footage and other recordings for the purposes described above.
